Entrant Application Form Waiver & Release From Liability
▪I/we hereby release The Havelock Country Music Talent Show Inc, havelocktalent.ca, its committee,
directors, agents, sponsors and staff, from any and all claims for damages, injuries, or loss of property
while participating in any activity related to this competition.
▪I/we understand and agree to abide by a standard of good sportsmanship and conduct my/ourselves
in a professional, appropriate manner at all times during the event.
▪I/we understand that I/we are not to approach the judges at any time and that the decision of
the judges is final.
▪I give permission to The Havelock Country Music Talent Show Inc. to use my/our likeness and
replications of performance’s to further promote the event.
▪My signature here, is my agreement to all the policies, rules & regulations set forth by The
Havelock Country Music Talent Show Inc.
Submitted along with this application must include the following:
- One CD only(3songs) of my/our performance for qualifying consideration. No DVD or Audio Cassette Tape
- A short biography which includes my/our goals as a country music performer.
- A photo suitable for publishing.
- $10.00 (ten dollars) non-refundable money order.
Entry deadline is July 4, 2008. Only those selected will be contacted.
